    How this mental health survey works

    This tool uses two of the most widely trusted mental health screening questionnaires in clinical use: the PHQ-9 for depression and the GAD-7 for anxiety. Both were developed and validated through peer-reviewed research and are commonly used by primary care doctors, therapists, and psychiatrists as a first step — not a final answer — in understanding someone's symptoms.

    You'll answer 16 questions one at a time, rating how often each symptom has bothered you over the past two weeks. At the end, you'll get a combined status report: your PHQ-9 and GAD-7 scores, a severity band for each, a breakdown of which symptom areas scored highest, and general suggestions for what to consider next.

    If any answer suggests thoughts of self-harm, a crisis-resources panel appears immediately — before you even finish the survey — because that matters more than a final score.
